ABSTRACT

Os tumores de glândula salivar (TGS) apresentam notável complexidade clínica e biológica, razão para a qual muitos estudos investigam os eventos envolvidos na sua progressão. Uma das dinâmicas envolvidas na invasão tumoral de diversos tipos de carcinomas é a transição epitélio-mesênquima (TEM). Neste processo, as células epiteliais sofrem transição para um estado mesenquimal móvel, favorecendo a invasão e metástase. Sendo assim, esta pesquisa analisou a expressão imuno-histoquímica de E-caderina, Twist1, Snail1, α-SMA, metaloproteinases de matriz 9 (MMP-9) e Vimentina (VM) em 90 casos de TGS, correlacionando-os entre si e com parâmetros clinicopatológicos. Foram selecionados 20 casos de Adenoma pleomórfico (AP), 20 casos de Carcinoma mucoepidermoide (CME), 20 casos de Carcinoma adenoide cístico (CAC), 10 casos de Adenocarcinoma polimorfo (ACP), 10 casos de Carcinoma epitelial-mioepitelial (CEME) e 10 casos de Carcinoma ex-adenoma pleomórfico (CexAP). A análise de E-caderina, Twist1, Snail1 foi realizada em parênquima tumoral sendo observado o percentual de células positivas (PP), com escores variando de 0 a 4, e a intensidade de expressão (IE), cujos escores variaram de 0 a 3. A avaliação de MMP-9 foi realizada em parênquima e estroma tumoral, também avaliando-se a PP e a IE, ambos baseados em escores que variaram de 0 a 3. A marcação para α-SMA e VM foi analisada em região de estroma tumoral. Células positivas para α-SMA foram contabilizadas em 10 campos, obtendo-se, então a média. A VM foi avaliada de forma qualitativa, utilizando-se 4 escores de acordo com a IE e se a marcação é difusa ou focal. Os dados obtidos foram analisados no software Statistical Package for Social Science, GraphPad Prism e STATA. O nível de significância de 5% foi adotado para os testes estatísticos. Foi verificada menor imunomarcação de E-caderina nos APs em relação às neoplasias malignas de glândula salivar (NMGS). Observou-se baixa imunoexpressão de Twist1 e Snail1 em APs. Em relação a expressão nuclear do Twist1, constatou-se maior expressão nas neoplasias malignas quando comparadas aos APs. Ainda, Twist1 em núcleo foi correlacionado à expressão citoplasmática de E-caderina nas NMGS. No que concerne aos parâmetros clinicopatológicos, esta proteína se relacionou estatisticamente com maiores chances de óbito. Foi evidenciada baixa imunoexpressão de Snail1 entre as NMGS. No entanto, na análise dos CACs, foi verificada maior expressão nuclear na variante sólida em relação às demais. A expressão de MMP-9 em parênquima demonstrou correlação positiva com Twist1 citoplasmático e Snail1nuclear nas NMGS. A MMP-9 também apresentou correlação positiva na comparação da sua imunoexpressão em região de parênquima e de estroma. A VM se apresentou como um biomarcador a ser considerado na avaliação clínica dos pacientes, já que esta apresentou relação significativa com tamanho do tumor (T3-T4) e maior frequência de óbito. Ademais, a alta expressão desta proteína se apresentou como um fator preditivo independente para piores taxas de sobrevida global (SG). A avaliação dos demais fatores clinicopatológicos apresentou estágios clínicos avançados como indicador de valor prognóstico independente para menores taxas de SG, enquanto que para a sobrevida livre da doença, estes foram a localização em glândula salivar menor e presença de metástase à distância. Os resultados deste estudo sugerem que o processo de TEM pode estar relacionado ao estágio de diferenciação celular em APs e à progressão tumoral nas NMGS. Ressalta-se, também, maior participação de Twist1 e MMP-9 no cenário da TEM em tumores malignos de glândula salivar, além da possibilidade de utilização da VM como indicador de valor prognóstico (AU).


Salivary gland tumors (SGTs) present remarkable clinical and biological complexity; therefore, many studies investigate the events involved in their progression. One of the dynamics involved in the tumor invasion of different types of carcinomas is the epithelial-mesenchymal transition (EMT). In this process, epithelial cells undergo a transition to a mobile mesenchymal state, favoring invasion and metastasis. Therefore, this research analyzed the immunohistochemical expression of E-cadherin, Twist1, Snail1, α-SMA, vimentin (VM) and matrix metalloproteinase 9 (MMP-9) in 90 SGTs cases; correlations among the biomarkers, as well as between the biomarkers and clinicopathological parameters were made. We selected 20 cases of pleomorphic adenoma (PA), 20 cases of mucoepidermoid carcinoma (MEC), 20 cases of adenoid cystic carcinoma (ACC), 10 cases of polymorphous adenocarcinoma (PAC), 10 cases of epithelial-myoepithelial carcinoma (EMC) and 10 cases of carcinoma ex-pleomorphic adenoma (CXPA). E-cadherin, Twist1, and Snail1 were analyzed in tumor parenchyma, observing the percentage of positive cells (PP) using scores ranging from 0 to 4, and the expression intensity (EI), whose scores were ranged from 0 to 3. The evaluation of MMP-9 was performed in tumor parenchyma and stroma, also evaluating PP and IE, both based on scores that ranged from 0 to 3. The labeling for α-SMA and VM was analyzed in stromal cells. Positive cells for α-SMA were counted in 10 fields and the mean was calculated. VM was evaluated qualitatively, using 4 scores according to EI and whether the labeling was diffuse or focal. Obtained data were analyzed using Statistical Package for Social Science, GraphPad Prism, and STATA software. The significance level of 5% was adopted for the statistical tests. Patients were mostly female, with a mean age of 49.8 years; the major salivary glands were the most affected anatomical site, mainly the parotid gland. A lower E-cadherin immunostaining was verified in PAs in comparison to malignant neoplasms of salivary glands (MNSGs). Low immunoexpression of Twist1 and Snail1 was observed in PAs. Regarding the nuclear expression of Twist1, it was found greater expression in malignant neoplasms than in PAs. Furthermore, Twist1 in the nucleus was correlated with cytoplasmic expression of E-cadherin in MNSGs. Regarding clinicopathological parameters, this protein was statistically related to higher chances of death. Low immunoexpression of Snail1 was evidenced among the MNSGs. However, in the analysis of CACs, greater nuclear expression was observed in the solid variant compared to the others. Expression of MMP-9 in parenchyma showed a positive correlation with cytoplasmic Twist1 and Snail1nuclear in MNSGs. MMP-9 also showed a positive correlation when comparing its immunoexpression in the parenchyma and the stroma. VM was presented as a biomarker to be considered in the clinical evaluation of patients since it showed a significant correlation between greater tumor size and a higher frequency of death. Furthermore, the high expression of this protein appeared as an independent predictive factor for worse overall survival (OS) rates. The evaluation of the rest of the clinicopathological factors showed advanced clinical stages as an indicator of independent prognostic value for lower rates of OS. For disease-free survival, these indicators were the location in the minor salivary gland and the presence of distant metastasis. Our results suggest that the EMT may be related to myoepithelial differentiation in PAs and tumor progression in MNSGs. Also, Twist1 and MMP-9 appear to play a greater role in the scenario of EMT in MNSGs; finally, VM might be used as a prognostic value indicator (AU).

ABSTRACT

O carcinoma secretório em glândula salivar é uma neoplasia recentemente descrita que tem os mesmos aspectos morfológicos, imuno-histoquímicos e genéticos do carcinoma secretório de origem mamária. O carcinoma secretório tem características celulares reminiscentes de uma célula secretora lactacional, isto é, um citoplasma vacuolado repleto de gotas lipídicas e um material secretado, por vezes de forma apócrina, que pode lembrar o leite. Mais recentemente, algum nível de diferenciação lactacional foi sugerida no carcinoma secretório de origem salivar. O objetivo do estudo foi verificar se existe uma diferenciação do tipo lactacional em carcinomas secretórios de origem salivar, comparando a outros tipos de carcinomas salivares mais comuns. Foram realizadas reações imuno-histoquímicas para as seguintes proteínas: receptores hormonais (receptor de prolactina e receptor do hormônio do crescimento), proteínas associadas ao produto de secreção da glândula mamária lactacional (mucina-1 (MUC-1), MUC4, globulina de gordura 1 do leite humano, lactoferrina) e proteínas associadas à via Akt-mTOR (PTEN, p-Akt, p-mTOR, p4EBP1, eIF4E, pS6). A maioria dos casos de carcinoma secretório foi negativa para receptor de prolactina e de hormônio do crescimento. Lactoferrina foi positiva em todos os grupos tumorais, porém somente em carcinoma secretório observou-se um padrão de marcação intensa, difuso tanto em célula como em secreção. Todos os casos de carcinoma secretório foram positivos para globulina de gordura do tipo 1, porém o mesmo padrão de marcação foi observado em outros tumores. A maioria dos casos de carcinoma secretório foram positivos para MUC1 e MUC4. Nenhum caso de carcinoma secretório foi positivo para Akt, mas PTEN foi difusamente expresso em 57,1% dos casos. mTOR foi expresso em mais da metade dos casos de carcinoma secretório e dos outros tumores salivares. Entre as proteínas à jusante de mTOR, somente eIF4E demonstrou alta expressão no grupo de estudo. A expressão de marcadores lactacionais não é exclusiva do carcinoma secretório, porém a expressão de lactoferrina é distinta neste grupo de tumores quando comparado aos demais tumores salivares estudados.

ABSTRACT

O siringoma condroide, também conhecido como tumor misto cutâneo, é uma neoplasia benigna rara, originada das glândulas sudoríparas, composta por estruturas epiteliais imersas em um estroma mixocondroide. Geralmente, apresenta-se como tumor sólido, único, localizado em face ou pescoço, com evolução crônica e assintomática. Relata-se caso de mulher, 75 anos, com lesão discretamente elevada na fronte, cujo diagnóstico foi definido pela análise histopatológica.


Chondroid syringoma, also known as a cutaneous mixed tumor, is a rare benign neoplasm originating from the sweat glands, composed of epithelial structures immersed in a myxochondroid stroma. It usually presents as a solid, single tumor located on the face or neck with a chronic and asymptomatic course. We report the case of a 75-year-old woman with a slightly elevated lesion on the forehead, whose diagnosis was defined by histopathological analysis.

ABSTRACT

ABSTRACT Mammary analogue secretory carcinoma is a rare neoplasm usually confused with other neoplasms in the salivary glands region. It has great similarity with the breast carcinoma. We report a case of a patient who presented with gingival submucosal bleeding and lesion, with the initial histopathological examination revealing salivary gland neoplasm of low crane. Computed tomography revealed the lesion near the tooth 27, with extension to the floor of the left maxillary sinus and to the palate mucosa. Resection of the infra-structure was performed, with a diagnosis of breast cancer secretory carcinoma in the minor salivary gland.

ABSTRACT

As neoplasias de glândulas salivares apresentam comportamentos diferenciados, que não seguem os padrões clássicos das neoplasias benignas e malignas. A raridade de algumas destas lesões dificulta ainda mais o entendimento dos mecanismos envolvidos na etiopatogenia. Marcadores moleculares como a proteína EZH2 têm sido utilizados na investigação de alterações epigenéticas em diferentes neoplasias, auxiliando na definição do diagnóstico e prognóstico das lesões. O objetivo do presente trabalho é avaliar a expressão da proteína EZH2 e descrever as características clínicas e microscópicas de amostras de carcinoma adenoide cístico (CAC) e adenoma pleomórfico (AP) com ênfase na importância da definição da malignidade da lesão. A análise dos cortes microscópicos corados em Hematoxilina e Eosina dos casos de Adenoma pleomórfico mostraram células epiteliais e mioepiteliais glandulares dispostas em lençóis e estruturas ductiformes em meio a estroma variável. Os casos de Carcinoma adenoide cístico mostraram três padrões distintos de crescimento incluindo formações tubulares, cribriformes e sólidas. Todos os casos de AP e CAC foram positivos para reação imuno-histoquímica para EZH2. As amostras de CAC apresentaram expressão de EZH2 significativamente maior comparado ao AP. As covariáveis metástase em linfonodos, recorrência, padrão histológico, presença de áreas sólidas e invasão perineural foram descritas em relação à marcação de EZH2 em amostras de CAC. Dessa forma, os resultados do estudo melhoram o entendimento das características clínicas e histológicas do CAC, assim como sobre o comportamento das lesões. Além disso, a análise mostra que o EZH2 é um potencial marcador de malignidade e ressalta a importância da validação de marcadores moleculares de alterações epigenéticas.


Salivary gland neoplasms present different behaviors, which do not follow the classic patterns of benign and malignant neoplasms. The rarity of some of these lesions makes it even more difficult to understand the mechanisms involved in the etiopathogenesis. Molecular markers such as the EZH2 protein have been used to investigate epigenetic changes in different neoplasms, helping to define the diagnosis and prognosis of the lesions. The aim of the study was to evaluate the expression of the EZH2 protein and to describe the clinical and microscopic characteristics of adenoid cystic carcinoma (ACC) and pleomorphic adenoma (PA) which emphasizes the importance of defining the malignancy of the neoplasm. Histopathological analysis of PA cases showed myoepithelial and glandular epithelial cells arranged as duct-like structures and sheets intermingled in the variable stroma and ACC cases showed the three growth patterns, tubular, cribriform and solid forms. All ACC and PA cases were positive for EZH2, with diffuse nuclear staining in neoplastic cells. The ACC samples showed significantly higher EZH2 expression compared to the PA. The covariables nodal metastasis, recurrence, growth pattern, presence of solid areas and perineural invasion have been described in relation to EZH2 staining in ACC samples. The results of the study improve the understanding of the clinical and histological characteristics of ACC, as well as on the behavior of lesions. In addition, the analysis showed that EZH2 is a potential marker of malignancy and highlights the importance of validating molecular markers of epigenetic alterations.

ABSTRACT

Salivary glands are important organs in the oral and maxillofacial region. Environment and genetic factors may cause salivary gland tumors or non-neoplastic diseases, but the mechanisms of those diseases are still unclear. One of the important reasons is the short of researching media and model. As a new technique and research model, organoids have been widely used in the research of various diseases. Organoid culture plays a bridging role between two-dimensional cell culture and living animal models, and it is also the most promising translational research model that could connect the clinical research to basic research. This review will discuss the recent development of organoid techniques in the culture of normal salivary glands and salivary gland tumors, also their applications and challenges in tissue engineering, etiological research, and tumor therapy.

ABSTRACT

In order to provide evidence-based recommendations on the management of salivary gland malignancy (SGM), the American Society of Clinical Oncology (ASCO) and the National Comprehensive Cancer Network (NCCN) formulated and updated SGM clinical practice guidelines respectively in 2021.The two guidelines are interpreted and compared in this article from six perspectives including preoperative evaluations, surgical diagnostic and therapeutic procedures, appropriate radiotherapy techniques, the role of systemic therapy, follow-up evaluations and recurrent-metastatic diseases' management of SGM, which has guiding significance for diagnosis and treatment of SGM in China.

ABSTRACT

Objective: To investigate the clinicopathological, immunophenotypic, and molecular genetic features of bronchial sialadenoma papilliferum (BSP). Methods: Four cases of BSP collected at the Shanghai Pulmonary Hospital from May 2018 to June 2021 were retrieved and analyzed. These cases were evaluated for their clinical, histological, immunohistochemical (IHC) and genomic features. The patients were followed up and relevant literature was reviewed. Results: All four patients were male, aged from 55 to 75 years (mean 62 years), with tumor diameter of 6 to 21 mm (mean 13.5 mm), and lesions were located in the left lower lobe (n=2), right lower lobe (n=1), and trachea (n=1). They were characterized by a combination of surface exophytic endobronchial papillary proliferation and an endophytic two-cell layered ductal structure. IHC staining showed that CK7 and EMA were strongly positive in ductal epithelium; p63, p40, CK5/6 were positive in ductal and papillary basal cells; SOX10 was positive in ductal epithelium and basal cells; S-100 was positive in basal cells and ductal epithelium in two cases. Next generation sequencing showed that two cases harbored BRAF V600E mutation. Conclusions: BSP is an extremely rare primary lung tumor arising from the salivary gland under bronchial mucosa. The primary treatment choice of this tumor is complete surgical resection. The diagnosis and differential diagnosis of this tumor depend on classic histomorphologic and IHC features, and BRAF V600E gene mutation can be detected.

Introducción: Las neoplasias de las glándulas salivales representan el grupo más heterogéneo y complejo de los procesos tumorales de la cabeza y el cuello. Objetivo: Caracterizar a los pacientes con neoplasias de glándulas salivales mayores que recibieron tratamiento quirúrgico en un hospital universitario cubano. Métodos: Se realizó un estudio observacional, descriptivo y transversal en pacientes diagnosticados histológicamente con neoplasias de glándulas salivales mayores. Las variables evaluadas fueron: edad, sexo, tipo de neoplasia, sitio primario, diagnóstico histológico y técnicas quirúrgicas. Resultados: Se estudiaron 55 pacientes con neoplasias, de las cuales 45 (81,82 por ciento) fueron benignas, con mayor frecuencia en los hombres (n = 28; 62,22 por ciento). La edad media de presentación de las neoplasias fue 55,11 ± 16,04 años, y el grupo de edad más afectado fue el de 40-59 años (n = 26; 47,27 por ciento). La parótida fue la glándula más afectada (n = 48), fundamentalmente por adenomas pleomorfos (n = 28; 58,33 por ciento). La parotidectomía subtotal fue la cirugía mayormente realizada (n = 38; 79,17 por ciento). Conclusiones: Las neoplasias parotídeas benignas presentadas en pacientes adultos del sexo masculino fueron las más frecuentes(AU)


Introduction: Salivary gland neoplasms are the most heterogeneous and complex group of head and neck tumoral processes. Objective: Characterize patients with major salivary gland neoplasms undergoing surgical treatment in a Cuban university hospital. Methods: A cross-sectional observational descriptive study was conducted of patients histologically diagnosed with major salivary gland neoplasms. The variables evaluated were age, sex, type of neoplasm, primary site, histological diagnosis and surgical techniques. Results: A total 55 neoplasms were studied, of which 45 (81.82 percent) were benign, with a higher frequency in men (n = 28; 62.22 percent). Mean age at neoplasm presentation was 55.11 ± 16.04 years, and the most affected age group was 40-59 years (n = 26; 47.27 percent). The parotid was the most frequently affected gland (n = 48), mainly by pleomorphic adenomas (n = 28; 58.33 percent). Subtotal parotidectomy was the most common surgical procedure (n = 38; 79.17 percent). Conclusions: Benign parotid gland neoplasms presenting in male adult patients were the most frequent type(AU)

Resumo Introdução: Tumores de glândulas salivares são um grupo diversificado de lesões, com várias origens e comportamentos extremamente diferentes, resultam em distintos desfechos para os pacientes. Portanto, a necessidade de descobrir novos marcadores com a capacidade de predizer o comportamento de neoplasias de glândulas salivares benignas e malignas é crucial. O syndecan-1 é uma proteína da superfície celular com papéis significativos em vários aspectos da função tumoral. Sua expressão nas neoplasias das glândulas salivares, especialmente seu componente estromal, ainda não foi investigada. Objetivos: Avaliar a imunopositividade do syndecan-1 nos componentes epiteliais e estromais das neoplasias de glândulas salivares e compará-la entre os subtipos benigno e maligno, além de avaliar sua correlação com os parâmetros clínico-patológicos. Método: Foram corados 133 tumores de glândulas salivares imuno-histoquimicamente com syndecan-1 e a intensidade e porcentagem dessa proteína foram determinadas, comparadas entre as lesões e correlacionadas com fatores clínico-patológicos. Resultados: A análise estatística das lesões com tamanho amostral suficiente mostrou diferenças significantes em porcentagem e intensidade entre os componentes epiteliais e estromais de todos os tumores (p < 0,05). As comparações pareadas demonstraram uma porcentagem de coloração significantemente maior das células epiteliais (p = 0,02) no tumor de Warthin em comparação com o adenoma pleomórfico e o carcinoma adenoide cístico. Da mesma forma, foram observadas intensidades de coloração e/ou percentagens significantemente maiores no carcinoma mucoepidermoide e no carcinoma adenoide cístico em comparação ao adenoma pleomórfico e ao tumor de Warthin (p < 0,05). Dos fatores clinico-patológicos, houve apenas uma correlação negativa significante entre o percentual estromal de carcinoma mucoepidermoide e a idade; e uma diferença significante entre a intensidade estromal + porcentagem de carcinoma adenoide cístico e sexo (p < 0,05). Conclusões: De acordo com nossos achados, o syndecan-1 estromal se correlaciona com o comportamento maligno de tumores de glândulas salivares, demonstra uma expressão mais alta, indica um papel para o syndecan-1 na invasão e metástase tumoral.

Introdução: O adenoma pleomorfo é a neoplasia benigna de glândula salivar mais comum, podendo acometer glândulas salivares maiores e menores. Essa neoplasia ocorre em uma ampla faixa etária, e se caracteriza por uma ampla variabilidade clinico-patológica e por altas taxas de recorrência. Objetivo: Relatar um caso de um adenoma pleomorfo, com enfoque nos aspectos clinico-patológicos, diagnóstico diferencial e manejo clínico. Relato de caso: Paciente do sexo masculino, 72 anos, exibiu lesão nodular, bem delimitada, localizada em fundo de sulco maxilar esquerdo, com tempo de evolução de quatro anos. A biópsia excisional foi realizada, e as hipóteses diagnósticas de hiperplasia linfoide, adenoma pleomorfo e lipoma foram consideradas, sendo esta última reforçada pelo fato de ter flutuado em formol. Sob análise microscópica, observou-se uma proliferação de células epiteliais e mioepiteliais em meio a um estroma variável. Com isso, o diagnóstico histopatológico de adenoma pleomorfo foi emitido. Conclusão: O presente estudo enaltece a importância da realização de análise histopatológica criteriosa para exclusão de outras hipóteses diagnósticas e neoplasias malignas. Dessa forma, a partir da associação dos achados clínicos e microscópicos, é possível obter um diagnóstico correto, guiando, consequentemente, na adoção de uma conduta terapêutica adequada(AU)


Introducción: El adenoma pleomórfico es la neoplasia benigna de glándulas salivales más común, que puede afectar a las glándulas salivales mayores y menores. Esta neoplasia se presenta en un amplio grupo de edad y se caracteriza por una amplia variabilidad clínica y patológica y altas tasas de recurrencia. Objetivo: Describir un caso de adenoma pleomórfico, con énfasis en aspectos clínicos y patológicos, diagnóstico diferencial y manejo clínico. Caso clínico: Paciente masculino, de 72 años, que presentó una lesión nodular bien definida ubicada en la parte inferior del surco maxilar izquierdo, con un tiempo de evolución de cuatro años. Se realizó una biopsia escisional. Se consideraron las hipótesis diagnósticas de hiperplasia linfoide, adenoma pleomórfico y lipoma, con mayor atención este último por el hecho de que flotaba en formaldehído. Bajo análisis microscópico, se observó una proliferación de células epiteliales y mioepiteliales en medio de un estroma variable. Por lo tanto, se emitió el diagnóstico histopatológico de adenoma pleomorfo. Conclusión: El presente estudio destaca la importancia de llevar a cabo un cuidadoso análisis histopatológico para excluir otras hipótesis diagnósticas y neoplasias malignas. Luego, con base en la asociación de hallazgos clínicos y microscópicos, es posible obtener un diagnóstico correcto, imprescindible en la adopción de un enfoque terapéutico apropiado(AU)


Introduction: Pleomorphic adenoma is the most common benign salivary gland neoplasm. It may affect major and minor salivary glands. It presents at a broad age range and is characterized by great clinical and pathological variability and high recurrence rates. Objective: Describe a case of pleomorphic adenoma, with emphasis on clinical and pathological aspects, differential diagnosis and clinical management. Case report: A male 72-year-old patient presents with a well-defined nodular lesion in the lower section of the left maxillary groove with a time of evolution of four years. Excisional biopsy was performed. The diagnostic hypotheses considered were lymphoid hyperplasia, pleomorphic adenoma and lipoma, with greater attention to the latter, due to the fact that it floated in formaldehyde. Microscopic examination revealed proliferation of epithelial and myoepithelial cells amidst a variable stroma. Thus, a histopathological diagnosis of pleomorphic adenoma was issued. Conclusion: The present study highlights the importance of careful histopathological examination to rule out other diagnostic hypotheses and malignant neoplasms. Next, clinical and microscopic findings will lead to an accurate diagnosis indispensable to adopt an appropriate therapeutic approach(AU)

El carcinoma del conducto salival es un tumor epitelial maligno agresivo, que involucra principalmente a la glándula parótida, con características histológicas semejantes al carcinoma ductal de glándula mamaria. El propósito de este trabajo fue presentar los resultados clínico-patológicos de cinco casos de carcinoma del conducto salival primario de glándula parótida y evaluar la expresión de Ki67. Histológicamente, el carcinoma del conducto salival presentó nidos epiteliales con patrones papilar, sólido y cribiforme, comedonecrosis tanto en la lesión primaria como en los nodos linfoides metastásicos y, además, invasión perineural. Se demostró con Ki 67 una alta proliferación celular en cuatro (80 %) de los cinco casos estudiados. Se concluyó que: el carcinoma del conducto salival es una lesión maligna de mal pronóstico, raramente informado en la literatura odontológica, con características histológicas semejantes a las del carcinoma ductal de alto grado de la mama; la comedonecrosis es un signo específico de esta enfermedad; puede desarrollarse "de novo" o en un adenoma pleomórfico preexistente; su diagnóstico diferencial histopatológico es fundamental para planificar su tratamiento y determinar su pronóstico, a pesar de su tratamiento quirúrgico y radioterapia postoperatoria es un tumor agresivo con alta proliferación celular, infiltración perineural, recurrencias y metástasis.


Salivary duct carcinoma is an aggressive malignant epithelial tumor, primarily involving the parotid gland, with histologic features similar to ductal carcinoma of the breast. The purpose of this work was to report the clinicopathological results of five cases of primary salivary duct carcinoma of the parotid gland and evaluate the expression of Ki67. Histologically, salivary duct carcinoma presented epithelial nests with papillary, solid, and cribriform patterns, with comedonecrosis in both the primary lesion and the metastatic limph nodes, and perineural invasion. A high cell proliferation was demonstrated with Ki67 in four (80 %) of the five cases studied. We concluded that: salivary duct carcinoma is a malignant lesion with a poor prognosis, rarely reported in the dental literature, with histological characteristics similar to those of high-grade ductal carcinoma of the breast; comedonecrosis is a specific sign of this disease; may develop "de novo" or in a pre-existing pleomorphic adenoma; its differential histopathological diagnosis is essential to plan its treatment and determine its prognosis; despite its surgical treatment and postoperative radiotherapy, it is an aggressive tumor with high cell proliferation, perineural infiltration, recurrences and metastasis.

ABSTRACT

It is described that salivary gland tumors account for up to 3-6% of head and neck tumors, 80% of these being located in the parotid gland. Broadly speaking, the smaller the gland, the greater the probability of malignancy. The most common clinical manifestation in tumor pathology is the appearance of a painless mass. Indicators of malignancy should be considered: chronic volume increase or rapid growth, facial paralysis, cranial nerve involvement, ulceration, and cervical lymphadenopathy. Ultrasonography should be the first imaging study, however, it provides limited information. MRI is the exam of choice for salivary gland tumors, whereas CT and PET-CT allow the study of local and remote invasion. FNA is controversial, it is not indicated for all patients, in parotid tumors a cytological examination is usually not necessary prior to surgery. Excisional biopsy is not recommended due to the risk of tumor seeding, neural damage, and salivary fistula. The goal of treatment in gland tumors is the removal of the glands, associated with lymph node dissection and adjuvant radiation therapy in indicated cases. The general practitioner must comprehensively address these pathologies to achieve a timely referral to the specialist. (AU)

ABSTRACT

As glândulas salivares são estruturas formadas por um sistema de ductos e ácinos responsáveis por secretar saliva. Apesar de raros, os tumores de glândulas salivares compreendem um grupo heterogêneo de lesões, apresentando diferentes características histológicas, sendo de difícil classificação e comportamento clínico diverso. A identificação de novos marcadores moleculares tem sido alvo de pesquisas para melhor compreensão e classificação dessas neoplasias, visto que a avaliação da expressão gênica e suas vias envolvidas permite identificar genes associados à regulação que modula o desenvolvimento neoplásico. Assim, novos achados podem direcionar a aplicação de novas técnicas para o diagnóstico, prognóstico e tratamento terapêutico. Contudo, pouco se sabe sobre a via de sinalização TGFß em neoplasias mais comuns em glândulas salivares, como: Adenoma Pleomórfico (AP), Carcinoma Mucoepidermoide (CME) e Carcinoma Adenoide Cístico (CAC). Diante disso, torna-se necessário ampliar a pesquisa de genes associados para a determinação de um painel de marcadores e, deste modo, fornecer informações que possam contribuir com o diagnóstico dessas neoplasias. O objetivo desse trabalho foi avaliar a expressão gênica relacionada à via de sinalização TGFß por meio da técnica de RT-PCR em tempo real (qPCR) destacando os marcadores TGFß1, ITGB6, SMAD2, SMAD4, FBN1, LTBP1 e c-MYC. Para tanto, foram selecionadas 13 amostras de AP, 17 de CME e 13 de CAC, além de 10 amostras de glândulas salivares não neoplásicas provenientes de cirurgias realizadas no A.C.Camargo Cancer Center no período do ano 2000 a 2015 e fornecidas pelo Biobanco de Tumores. Os resultados indicam que em pacientes com AP há aumento da expressão dos genes TGFß1, LTPB1, c-MYC e FBN1, enquanto a expressão de SMAD2 diminui quando comparados às amostras não neoplásicas. Em pacientes com CME, foi observada expressão aumentada dos genes TGFß1, ITGB6, FBN1 e c-MYC enquanto a expressão dos genes SMAD2 e SMAD4 diminui ao serem comparados às amostras não neoplásicas. Nos pacientes com CAC, foi observada expressão aumentada em quase todos os genes avaliados. Na análise de clusterização hierárquica não foi possível classificar nas diferentes neoplasias de glândula salivar. Para a validação dos resultados de expressão gênica foi realizada uma meta-análise utilizando dados da literatura, sendo possível observar concordância nos valores de expressão dos genes ITGB6, LTBP1 e TGFß1 em amostras de CME e dos genes FBN1, ITGB6, LTBP1, c-MYC, SMAD2 e SMAD4 nas amostras de CAC. Comparando-se a expressão dos genes entre os três tipos de neoplasias estudados, foi observado aumento de expressão dos genes c-MYC, SMAD2 e SMAD4 nos casos de CAC e aumento da expressão do gene ITGB6 nos casos de CME. A análise de sobrevida demonstrou que, em pacientes com Carcinoma Mucoepidermoide foi observado que a ausência de linfonodo comprometido e ausência de recidiva estão associadas a melhor probabilidade de sobrevida global em 5 anos. Nossos resultados sugerem que a expressão diminuída dos genes SMAD2 e SMAD4 parece não interferir na regulação transcricional de c-MYC, especialmente no AP e CME. Considerando os genes ITGB6, TGFß1, LTBP1, FBN1 e c-MYC a expressão aumentada parece ser relevante para a regulação da via de sinalização no processo de tumorigênese. Sendo assim, este estudo contribui para um melhor entendimento da via de sinalização TGFß em neoplasias de glândulas salivares, além de fornecer informações para o desenvolvimento de potenciais marcadores biológicos para essas neoplasias.


Salivary glands are structures formed by a system of ducts and acini responsible for secreting saliva. Although rare, salivary gland tumors comprise a heterogeneous group of lesions, presenting different histological features, difficult classification, and diverse clinical behavior. Identification of new molecular markers has been the subject of researchers for better comprehension and classification of these tumors, since gene expression evaluation and their signaling pathways allow the identification of genes associated with regulation that modulated tumor development. Therefore, new findings can direct the application of new technologies for diagnosis, prognosis, and therapeutic treatment. However, little is known about the TGFß signaling pathway in the most common salivary gland tumors, such as: Pleomorphic adenoma (PA), mucoepidermoid carcinoma (MEC), and adenoid cystic carcinoma (ACC). In addition, it is necessary to expand research of genes and associated genes for determining a panel of markers and, thus, provide information that could be contribute with the diagnostic of these neoplasms. The aim of this study is to evaluate the expression of genes associated with the TGFß signaling pathway by real-time RT-PCR (qPCR) highlighting the markers TGFß1, ITGB6, SMAD2, SMAD4, FBN1, LTBP1, and c-MYC. For this purpose, 13 PA samples, 17 MEC samples, 13 ACC samples, and histologically normal salivary glands samples were selected from surgeries performed at A.C.Camargo Cancer Center between 2000 and 2015. These samples were provided by Tumor Biobank. The results indicate that PA patients presented an increased TGFß1, LTPB1, c-MYC, and FBN1 gene expression whereas SMAD2 expression was decreased when compared to the normal samples. In MEC patients, increased expression of TGFß1, ITGB6, FBN1, and c-MYC genes was observed whereas SMAD2 and SMAD4 genes presented decreased expression. In ACC patients, increased expression in almost all genes was observed. In hierarchical clustering analysis it was not possible to classify the different salivary gland tumors. For the validation of the gene expression results it was carried out a meta-analysis using the literature date, being possible to observe an agreement in the expression values of the genes ITGB6, LTBP1 and TGFß1 in MEC samples and FBN1, ITGB6, LTBP1, c-MYC, SMAD2 and SMAD4 in ACC samples. Comparing gene expression among the three tumor types studied it was observed higher expression of c-MYC, SMAD2 and SMAD4 genes in ACC cases and higher expression of ITGB6 in MEC cases. Survival analysis demonstrated that, in MEC patients it was observed that absence of affected lymph nodes and absence of recurrence are associated with better overall survival in 5 years. Our results suggest that the decreased expression of SMAD2 and SMAD4 genes seems not to interfere with the transcriptional regulation of c-MYC, especially in PA and MEC. Considering ITGB6, TGFß1, LTBP1, FBN1 and c-MYC increased gene expression appears to be relevant for the regulation of the signaling pathway in tumorigenic process. Thus, this study contributes to a better understanding of TGFß signaling pathway in salivary gland tumors, apart from supplying information in development of potential biomarkers for these tumors.

Introduction: The tumors affecting salivary glands have a wide morphological diversity. Objective: This study aimed to examine the prevalence of salivary gland tumors in patients treated at São Lucas Teaching Hospital at the Pontifical Catholic University of Rio Grande do Sul (HSL-PUCRS), in Porto Alegre (RS), Brazil, from 2007 to 2016. Method: A retrospective study analyzing 201 files from the Department of Pathology at the HSL-PUCRS was carried out, by revising the medical records. Results: Seventy-three cases of salivary gland tumors were found, and their electronic and physical medical records were analyzed. Of the 73 cases, 56 (76.7%) were benign tumors and 17 (23.3%) were malignant tumors. The age group with the highest number of cases was between 41 and 60 years of age and the highest prevalence was found in females, with 54.8% of the cases. The parotid gland presented the highest prevalence, accounting for 72.6% of the cases. The predominant neoplasia was the pleomorphic adenoma, accounting for 53.4% of the tumors. The standard of distribution of neoplasms of salivary glands was similar to the encountered in other Brazilian regions. Conclusion: The largest salivary glands were the most affected by neoplastic processes. Pleomorphic adenoma and adenoid cystic carcinoma were the most frequent benign and malignant tumors, respectively, and parotid gland was the most affected site. In the light of previous literature data, the results allow to infer that some demographic characteristics (for example, sex and age) vary among the different geographic regions


Introducción: Los tumores que afectan a las glándulas salivales tienen una amplia diversidad morfológica. Objetivo: Identificar la prevalencia de neoplasias de glándulas salivales en pacientes atendidos en el Hospital São Lucas da Pontifícia Universidade Católica do Rio Grande do Sul (HSLPUCRS), en Porto Alegre (RS), desde 2007 hasta 2016. Método: Estudio retrospectivo mediante el análisis de 201 registros del Departamento de Patología de HSL-PUCRS. Resultados: Se encontraron 73 casos de neoplasias de glándulas salivales y se analizaron los registros electrónicos y físicos de los casos seleccionados. De los 73 casos, 56 (76,7%) fueron de neoplasias benignas y 17 (23,3%) de neoplasias malignas. El grupo de edad con mayor número de casos fue el de 41 a 60 años, y la mayor prevalencia en mujeres, con 54,8%. La glándula parótida tuvo una mayor prevalencia, constituyendo 72,6% de los casos. El tipo neoplásico más prevalente fue el adenoma pleomorfo, con 53,4%. El patrón de distribución de las neoplasias de glándulas salivales fue similar al encontrado en otras regiones de Brasil. Conclusión: Las glándulas salivales mayores fueron las glándulas más afectadas por procesos neoplásicos. El adenoma pleomórfico y el carcinoma adenoide quístico fueron los tumores benignos y malignos más frecuentes, respectivamente y el sitio más afectado fue la glándula parótida. Con base en la literatura previa, estos resultados permiten inferir que algunas características demográficas (por ejemplo, sexo y edad) varían entre las distintas regiones geográficas

Guimarães Os vírus têm sido considerados como importantes patógenos no desenvolvimento de neoplasias em glândulas salivares, dentre estes, destacam-se os Betaherpesvirus humano como o Citomegalovirus humano (HCMV), Herpesvirus humano 6 (HHV-6) e Herpesvirus humano 7 (HHV-7). Os betaherpesvírus são característicos por apresentarem alta prevalência na população mundial, sem apresentar sazonalidade, capazes de causar infecção latente e reativação viral em seus hospedeiros. Devido a detecção destes vírus em amostras de saliva, tem sido proposto em alguns estudos a ação de betaherpesvírus em algumas patogenias de glândulas salivares, como a formação de patogenias nestes órgãos. Neoplasias em glândulas salivares representam cerca de 3-6% de todas as neoplasias de cabeça e pescoço, com incidência mundial anual de aproximadamente 0,4-13,5% por 100.000 indivíduos. Apesar de haver dados de detecção de betaherpesvírus em neoplasias salivares e em amostras de saliva, ainda existem estudos insuficientes que explorem o papel destes vírus nestas patogenias salivares. O objetivo deste estudo foi investigar a presença dos Betaherpesvirus humano (HCMV, HHV-6 e HHV-7) em neoplasias de glândula salivar parafinadas. Um ensaio de qPCR foi realizado para amplificação das regiões U54, U56 e U37 do HCMV, HHV-6 e HHV-7, respectivamente para quantificar a carga viral em 68 amostras parafinadas de lesões salivares. Dentre as 68 amostras processadas, 51,4% eram de mucocele (35/68), 39,7% de adenoma pleomórfico (27/68) e 8,8% de carcinoma mucoepidermoóide (6/68). A detecção de betaherpesvírus nestas lesões foi alta, apresentando maior detecção para HCMV com 52,9%, 47,05% para HHV-6 e 39,7% para HHV-7, possuindo predominância de detecção de betaherpesvírus na lesão do tipo adenoma pleomórfico. Foi observado que 50,0% das amostras apresentaram tripla-infecção por HCMV/HHV-6/HHV-7, sendo detectado 20,0% de coinfecções por HCMV/HHV-6, 20,0% de HCMV/HHV-7 e 10,0% de HHV-6/HHV-7, com coinfecções ocorrendo na lesão do tipo adenoma pleomórfico em maior taxa. A alta detecção de HCMV, HHV-6 e HHV-7 em glândulas salivares, indica que este órgão pode ser possível de sítio de replicação destes vírus


The viruses have been considered as important pathogens in the development of neoplasms in salivary glands, between these, Human betaherpesvirus such as Human cytomegalovirus (HCMV), Human herpesvirus 6 (HHV-6) and Human herpesvirus 7 (HHV-7) stand out. Betaherpesvirus is characteristic because they have a high prevalence in the world population, without presenting seasonality, capable of causing latent infection and viral reactivation in their hosts. Due to the detection of these viruses in saliva samples, the action of betaherpesvirus in some pathogenesis of salivary glands, such as the formation of pathogenesis in these organs, has been proposed in some studies. Salivary gland neoplasms account for about 3-6% of all head and neck neoplasms, with an annual worldwide incidence of approximately 0,4-13,5% per 100,000 individuals. Although there are data for the detection of betaherpesvirus in salivary neoplasms and saliva samples, thereare still insufficient studies exploring the role of these viruses in these salivary pathogeneses. The aim of this study was to investigate the presence of human Betaherpesvirus (HCMV, HHV-6 and HHV-7) in paraffin salivary gland neoplasms. A qPCR assay was performed to amplify the U54, U56 and U37 regions of HCMV, HHV-6 and HHV-7, respectively to quantify the viral load in 68 paraffin samples of salivary lesions. Among the 68 samples processed,51,4%were mucocele (35/68), 39.7% pleomorphic adenoma (27/68) and 8,8% mucoepidermoid carcinoma (6/68). The detection of betaherpesvirus in these lesions was high, presenting higher detection for HCMV with 52,9%, 47,05% for HHV6 and 39,7% for HHV-7, with predominance of detection of betaherpesvirus in the lesion of the pleomorphic adenoma type. ABSTRACT Objective: To determine the association of socio-demographic and clinic-pathological risk factors with oral cancer in Kelantan, Malaysia. Material and Methods: A 19-year cross-sectional survey was performed in Hospital Universiti Sains Malaysia (HUSM), Malaysia. Medical record of 301 oral cancer patients was retrieved from the Medical Records office. Results: The majority of the oral cancer cases were male (62.8%), non-smokers (57.5%), non-alcohol consumers (83.4%), non-betel quid chewers (96.7%), and belonged to Malay ethnicity (68.8%). At the time of diagnosis, most of the patients were at stage II (38.9%). Approximately one-third (30.6%) of the total OC patients experienced loco-regional/distant metastasis, whereas no metastasis was detected in around two-thirds of cases (69.4%). A combination of surgery and radiotherapy was the most commonly employed treatment modality (27.2%). At the time of this study, the survival status of most of the patients was alive (69.1%). The most frequently encountered oral cancer in the Kelantanese population was oral squamous cell carcinoma (70.1%), with the tongue being the most frequently involved oral cavity site (35.5%). Conclusion: More than three-fourths of the cases were alive at follow-up, which included the cases that did not undergo any form of treatment.

ABSTRACT

Necrotizing sialometaplasia (NS) is a benign, self-limiting inflammatory entity that mainly affects the minor salivary glands located in the hard palate. Classically, NS is characterized as a nodule that evolves to a central ulcer. The most widely recognized triggering factor is an ischemic event. The diagnosis becomes a challenge in non-ulcerated NS cases which is essential to rule out the possibility of salivary gland tumors, especially the malignant ones. Here, we presented a case of a 32-year-old male patient with a 1-month complaint of a painful, slightly elevated erythematous area on the hard palate. Incisional biopsy was performed, and NS was diagnosed based on histopathological and immunohistochemical analyses. Clinicians should be aware of and consider NS as a differential diagnosis of minor salivary gland tumors, particularly when it presents as a non-ulcerated clinical aspect.

ABSTRACT

El carcinoma de conductos salivales (CCS) es una neoplasia maligna primaria clínica y patológicamente distinta de las glándulas salivales. Debido a su rareza existe una falta de documentación exhaustiva en la literatura con respecto a sus características, manejo y resultados clínicos. En la presente publicación se presenta un caso de un paciente de sexo masculino de 74 años, que acudió al servicio de odontología del Hospital Interzonal General de Agudos Presidente Perón de Avellaneda. El resultado ecográfico arrojó a nivel submaxilar derecho una imagen nodular de aspecto solido que mide 25 x 24 mm Se le realizó la intervención quirúrgica y el resultado anatomopatológico confirmó el diagnóstico de carcinoma de conductos salivales, una de las neoplasias salivales más agresivas. En la actualidad, la muerte ocurre en 60 a 80% de los pacientes, por lo general dentro de los 5 años; alrededor del 33% desarrolla recidiva local y más del 50% metástasis a distancia, en sitios que incluyen pulmones, huesos, hígado, cerebro y piel (AU)


Salivary duct carcinoma (SDC) is a clinically and pathologically distinct primary malignant neoplasm of the salivary glands. Due to its rarity, there is a lack of exhaustive documentation in the literature regarding its characteristics, management, and clinical results. This publication presents a case of a 74-year-old male patient who attended the dentistry service of the Interzonal General Acute Hospital President Perón by Avellaneda. The ultrasound result revealed a nodular image at the right submaxillary level solid aspect measuring 25 x 24 mm. Surgical intervention was performed, and the pathological result confirmed the diagnosis of salivary duct carcinoma, one of the most aggressive salivary neoplasms. Currently, death occurs in 60% to 80% of patients, usually within 5 years; about 33% develop local recurrence and more than 50% distant metastases, at sites including the lungs, bones, liver, brain, and skin (AU)

Abstract The aim of this study was to identify tumor parenchyma cells exhibiting immunohistochemical profile of stem cells by evaluating the immunoreactivity of OCT4 and CD44 in a number of cases of salivary gland neoplasms. The sample consisted of 20 pleomorphic adenomas, 20 mucoepidermoid carcinomas, and 20 adenoid cystic carcinomas located in major and minor salivary glands. The expression of OCT4 and CD44 was evaluated by the percentage of positive cells and the intensity of expression. All studied cases showed positive expression of OCT4 and CD44 and higher values than the control groups. For OCT4, luminal and non-luminal cells were immunostained in the case of pleomorphic adenomas and adenoid cystic carcinomas. Moreover, the immunoreactivity of CD44 was particularly evident in the non-luminal cells of these lesions. In mucoepidermoid carcinomas, there was immunoreactivity for both markers in squamous and intermediate cells and absence of staining in mucous cells. For both markers, a significantly higher immunostaining was verified in neoplasms located in the major salivary glands compared with lesions in minor salivary glands (p<0.001). In the total sample and in minor salivary glands, malignant neoplasms exhibited higher immunoreactivity for OCT4 than pleomorphic adenoma. A significant moderate positive correlation (r = 0.444 and p ≤ 0.001) was found between OCT4 and CD44 immunoexpression in the total sample. The high expression of OCT4 and CD44 may indicate that these proteins play an important role in identifying tumor stem cells.
